  7. 2 pcs. 1.5 x 1.5 ID Pictures
  8. Application for Admission Form
  9. P 300.00 Registration Fee (non-refundable)
  10. Long Brown Envelope / Long Plastic Envelope
  11. CEIS-MAL-RO-004 (if Transferee)
