Admission Procedure

FOR CEIS MANILA

ADMISSION PROCEDURES FOR INCOMING GRADE 11 STUDENTS

  1. Complete the information in the system/ online (APPLY HERE)

  2. Print dully accomplished Admission Form & Consent Form.

  3. Pay the reservation fee at the Cashier department.

  4. Proceed to Senior High School Office located at MVH Lanai for checking and submission of Application form and to get schedule of enrollment.

  5. Prepare the following requirements for enrollment:

    1. Consent Form (Applicant for Enrollment and Student)

    2. Certificate of Good Moral Character

    3. Form 138 (Original)

    4. Birth Certificate

    5. Photocopy of JHS Completion Certificate

    6. ESC/QVR Certificate (if available)

 

Additional requirements for Non Resident Applicants:

  1. Certificate of Evaluation from DepEd-SDO Manila

  2. Photocopy of Passport

  3. Police Clearance

 

CEIS – MAKATI (GIL PUYAT CAMPUS)

REGISTRATION PROCEDURE

FOR ONLINE APPLICATION:

  1. Complete the information in the system online. (APPLY HERE)
  2. Print duly accomplished admission form and consent form.
  3. Pay Php300 at the Accounting/Cash Section located at the ground floor.
  4. Submit the accomplished application form to the Office of the Assistant Principal located at the 5th Floor.
  5. Wait for the announcement of the reservation schedule on our Facebook page, Centro Escolar University – Makati.

 

FOR WALK-IN APPLICATION:

 

  1. Get the application form from the Office of the Assistant Principal located at the 5th floor.
  2. Fill-out the registration form and attach a 1.5x 1.5 picture.
  3. Pay Php300 at the Accounting/Cash Section located at the ground floor.
  4. Submit the accomplished application form to the Office of the Assistant Principal.
  5. Wait for the announcement of the reservation schedule on our Facebook page, Centro Escolar University – Makati.

RESERVATION PROCEDURE

  1. Present the grade 10 report card (up to 3rd quarter only) and good moral certificate to the encoder.
  2. Secure a reservation slip.
  3. Pay Php5,000 at the Accounting/Cash Section.
  4. Wait for the announcement of the schedule of enrolment on our Facebook page, Centro Escolar University – Makati.

 

CEIS – MALOLOS

REGISTRATION PROCEDURE

ADMISSION PROCEDURES FOR INCOMING GRADE 11 STUDENTS

  1. Complete the information in the CEIS SHS Online Application (APPLY HERE)

  2. Print duly accomplished Admission Form & Consent Form.

  3. Pay the reservation fee at the CEIS Cashier (Php 300.00)

  4. Proceed to Senior High School Office located at Asst. Principal's Office PHLim Bldg. CEU Malolos for checking and submission of Application form and to get a schedule of enrollment.

  5. Prepare the following requirements for enrollment:

    1. Consent Form (Applicant for Enrollment and Student

    2. Form 138 (Card)

    3. ESC/Voucher Certificate (For incoming Grade 11)

    4.  School Certification e.g. Good Moral/ Completion/ Ranking for Rank 1-3 with 100 and above JHS Completers for Incoming Grade 11 applying for Scholarship

    5.  PSA Birth Certificate

 

Additional requirements for Non Resident Applicants:

  1. Certificate of Evaluation from DepEd-SDO Malolos (to be endorsed by the Principal/Registrar) graduates of foreign schools.

  2. Photocopy of Passport/Alien certificate/VISA if foreign national or dual-citizen

  3. Police Clearance

NOTE:

*If the report card reflects grades up to 2nd quarter only or semestral, secure a certification that school started the academic year later than June or has a semestral scheme.

**Reservation fee is non-refundable and transferrable but deductible from the total tuition fee upon enrolment.
